1996 Avia Air Accident occurred MAR-13-96 at PUNTO FIJO

Remarks: On March 13, 1996, about 0815 mountain daylight time, a Cessna 402B, P4AVB, registered to Avia Air, operating as a scheduled passenger
flight crashed about 16 nautical miles north of Las Piedras Airport, Punto Fijo, Venezuela. Visual meteorological conditions prevailed and a VFR flight
plan was filed. The commercial pilot and seven passengers were fatally injured. The flight originated from Reina Beatrix Airport, Aruba, about 20 minutes
before the accident. Another pilot from the same company departed Reina Beatrix Airport behind the accident airplane. While en route the pilot of P4AVB
contacted the other pilot and stated he was having a mechanical problem with the left engine. A short time later he reported a left engine fire, which was
visually verified by the other pilot. An outboard section of the left wing and the left engine separated from the airplane, and the airplane crashed north of
the Las Piedras Airport. The investigation is under the jurisdiction of the Government of Venezuela, and was delegated to the Government of Aruba. One
American citizen was fatally injured.

Remarks: The no.3 propeller gearbox failed, causing the prop to separate. A broken propeller blade struck the no.4 propeller, which also separated. A
propeller then crossed under the aircraft slicing through the fuselage, cables, hydraulics and pressure vessel, damaging the no.2 engine. The no.2, 3
and 4 engines were shut down. An emergency landing was carried out at Aruba with no brakes, 10deg flaps and limited flight controls.
Probable Cause: "The in-flight loss of the nos 3 and 4 propellers and gearboxes for undetermined reasons. The failed components were not recovered
from the Caribbean sea."

1997 B737 14 CFRPart 121 operation of Air Carrier RYAN INTERNATIONAL AIRLINES

On January 18, 1997, about 1731 eastern standard time, a Boeing 737-400, Irish registration EI-CNF, registered to Aer Lingus, and operated by Ryan
International Airlines, as a Title 14 CFR Part121 nonscheduled international passenger flight, from Aruba to Cincinnati, Ohio, had a passenger injured
during an emergency evacuation after returning to Aruba after departure. Visual meteorological conditions prevailed at the time and an instrument flight
rules flight plan was filed. The aircraft received minor damage and the airline transport-rated pilot, first officer, extra pilot, 5 flight attendants, and 148
passengers were not injured. One passenger received serious injuries. The flight had originated from Aruba, the same day, about 1418. The captain
reported to the operator that during the takeoff roll, the No. 1 tire failed about 5 knots before V1. He continued the takeoff and left the landing gear down.
Ground personnel reported they found tire debris and some metal debris on the runway. He flew by the tower several times and they confirmed the No. 1
tire had failed. They flew around for about 3 hours and burned off fuel. They made an uneventful emergency landing. As planned, after landing, they
initiated an emergency evacuation of the aircraft through the right side exits away from the damaged tire and wheel. The right front slide did not deploy
properly and they sent the forward passengers through the left front exit.
